Job Description:
Responsibilities
Monitors clinical progression, reassess and adjusts plan of care accordingly. Utilizes the nursing process to assess, plan, implement and evaluate the discharge needs for patients in assigned caseload. Performs utilization management activities using approved guidelines. Provides follow up with appropriate resource for utilization management issues/non-compliance. Participates in quality improvement (QI) activities. Advances knowledge and skills in case management. Collaborates effectively with departmental staff to achieve goals and objectives.
Four year college degree in Social Work , Human Services, or related degree required. Masters in Social Work preferred. 3-4 years experience in hospital discharge planning, social work, case management, home health, DSS, long term care facilities or insurance case managed setting required. MSW may be substituted for two years of required experience or a field placement in a healthcare setting may be substituted for one year of required experience.
