Job Description:

Active Supplemental Days Associates Degree Required The position exists to manage clinical outcomes, cost, and customer satisfaction in the daily care assignment and in the operations of the unit. The incumbent assures care for patients and families reflective of Clarian values. This position coordinates the nursing and interdisciplinary team for daily patient care assignments. This position provides direct and indirect client care services primarily delivered on a visiting and intermittent basis in the client's home in accordance with policies and procedures. Performs initial and ongoing nursing assessments of the patient and family. Triages clients, and performs follow-up and referral for coordinated client care management. Works independently and is required to make judgments recognizing that health/illness is a continuum, recognizing safety in the home environment, and recognizing symptoms of high risk situations to prevent rehospitalization in accordance with established policy. Utilizes expertise to help reestablish a client's optimal health. Focus on promoting health to assist the patient to modify their self-care skills according to their disease process. Participates in after hours and weekend home visits and on-call telephone triage as required. Activities include health promotion, disease and disability prevention, restorative care and care of the acutely and/or terminally ill. The Home Care Nurse functions as an essential member of the health team with the community.
QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES

Requires current state of Indiana license as Registered Nurse.
Basic Life Support certification required. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
Requires ability to provide clinical reasoning; decision-making and clinical thinking.
Requires ability to promote compassion and a therapeutic environment.
Requires ability to provide multiple forms of communication (presence; touch; eye contact; etc . ) Requires ability to recognize and appreciate cultural diversity at many levels.
Requires ability to provide high level of patient confidentiality.
Requires strong written and verbal communications skills at the staff; patient and family levels.
Requires ability to actively seek educational opportunities to improve clinical practice.
Requires ability to maintain quality-working relationship with peers.
Requires one to two years experience in medical/surgical and/or geriatric nursing.
Experience in public health or home care is helpful.
Requires understanding and ability to demonstrate general nursing skills.
Requires ability to work independently.
